1756-IF4XOF2F Module Fault

Hi All,

I am using 1756-IF4XOF2F module in Controllogix hardware. I have got one situation here now. I am getting 6 volts at all 4 channels of module.
Also I have attached moduel's fault status images.
Plz go through images and suggest me what can I do.

Looks like a wiring problem....the input is reporting 11.5 or slightly over 10V, so your overrange and Hi limit alarms are set.
