formulas for calculating transformer

I am looking for the formulas to caculate transformer kva the fuse rating for single phase and 3phase input and single phase output
example if I had a 3ph.input of 220volts and an output of 110v 1ph
15 amp what would be the kva,how many amps would it draw at 220v 3ph, and how should i caculate the fuse protection .
