Measuring Voltage and Amps on a rectifier

Hi all,
I need to bring Volt and Amp signals from a large rectifier into an analog card on a PLC.
Do you know of any devices "shunts", "current transformers", voltage transformers", etc. that can handle 250A and 400V. Preferrably I would like to use 4-20mA signals to the PLC.
This application is for monitoring Volts amd Amps used in a plating process.
