Fanuc alarm Tachogenerator Disconnect

Hy guys, we have a CNC with Fanuc controller (motors,…). Yesterday, we had an alarm “Tachogenerator Disconnect” on one of the axis (with servomotor). There is an encoder (type A860-0304-T011 2000P) on the motor (type A06B-0512-B001), where did the tacho come from? What does this alarm mean? Has anyone encountered something like this before? We checked all connectors and cables without any result. The manuals are not helping, and the machine is really old to find spare parts easily. Thanks
 
is this a vertical or horizontal machining center? If so the sensor for the spindle orientation could be bad. Also on some lathes the sensor is built inside the spindle motor. What kind of machine and what kind of Fanuc control will be helpful.
