How can i connect Siemens PLC with modbus protocol?

That is a MAC address, more or less equivalent to a serial number of a particular individual PLC.

What was asked for was the model number, which usually starts with 6E e.g. something like this (source: ebay.com):

Do you mean you have an ET200SP mounted S7-1510 or 1512, or is this just an ET200 remote IO with a separate PLC?

If it's the first one, yes this should allow you to communicate via Modbus TCP over the onboard ethernet port. You can do this with all S7-1200 and S7-1500 CPU's as far as I know.
