RSlogix 500 Branch output problems

I am having problems with my branch outputs both turning on it will only turn on one output that is output 4? not sure what I am doing wrong.

I:0/2 O:0/1

O:0/4

Is there something else later in the program that is also addressing O:0/1?

It does not matter what I title the output it will still only activate output 4.

It may be possible that output 4 is being turned on by some other logic and this rung is not being activated at all. If you can, zip and post the program.
