mA Output Freezes at 16mA

Output stops when it reaches 16mA (75%).

0% -> 0mm -> 4mA
25% -> 2400mm -> 8mA
50% -> 4800mm -> 12mA
75% -> 7200mm -> 16mA
100% -> 7200mm -> 16mA

Happens at PLC side.
Do you experience or encounter the same problem?

Output from Yokogawa DCS were simulated, works properly from 0% to 100%.
