Submodule electrical fault

I'm attempting to blow an S5 program onto an eprom. We ordered it in, I did a read which came up empty, so I hit f1, selected the module from the list to ensure I had the correct prog #, however I got error 1414: Submodule electrical fault.

I did a UV erase just to ensure the the module is actually empty but I'm getting the same error.

Any ideas what could have caused this, or how to correct it?
