SensaGuard Rectangular Safety Sensor- Random Faults, Any thoughts ?

Good Afternoon ,

I just finished a project using Rockwell SensaGuard Safety Sensors. I keep getting random faults …… Red Flashing LED . Manual says the flashing red LED
OSSD -Check OSSD Outputs are shorted.

Since they are sold in a pair , you don't need to teach them or anything , do you ? The number on it is 440N-Z21SS2H.

They are wired correctly to a Rockwell 440C-CR30 Safety Relay
