1756-IB16S 16 channel safety input card for 5580 guardlogix controller issue?

Hi all,
We tried to setup the hardware onto a 10 slot rack as follows,
PS
Slot 0: 5580 controller
Slot 1: Safety Partner
Slot 2: Safe Inp
Slot 3: Safe Out
Slot 4: Network comms card
Slot 5-9 empty

So here's the issue we've been getting.
All parts are out of box.
Safety output card has a solid green OK and we're able to read the module info and reset ownership.
Safety input card has flashing orange and green OK. And can't get module info or reset ownership.
Tried different slots but no luck.

Can someone please she'd some light on the issue.?

Card must go in the slot you set up for it in the hardware tree???
Are the inputs configured in the module?
Is your safety network number consistent? (Not sure if that's a thing for local I/O)
DO YOU HAVE an error mesage and code in the module properties?
 
Manual does not list "flashing orange/green"
It does say that "flashing red/green" means you must commission the Unique Node Identification Number.
Does the yellow yield sign show by the module in the hardware tree?

Yes that's what's happening. How do we commission the UNID number.?

address is the slot number of the device."

Like I asked before, is your safety network number consistent?
You can find this number in the safety processor properties, I think.
Or the Output module properties.
Then enter or paste into the Input module properties.
Reset the module first, I think.

I haven't run into this UNID thing before, but I think it basically just means you need the SNN to match the Safety PLC and other modules. And reset the module if you change slots. Prolly a new-out-of-the-box module would create its own setup when it is configured, but this one had issues.

Yes we have come to the conclusion it's a dodgy card.
We got swapped it from one slot to the other, it works for 10 seconds with solid green OK and then gets lost in communication timeout errors.
We're organising a replacement part.
