Do I need to make a DP15 connector cable to connect to it and my PLC?

That's a feedback connector kit for TLY-series servomotors.

You connect the servomotor's encoder feedback cable leads to the screw terminals inside the connector, and plug the connector into a Kinetix 2000 or Kinetix 6000 motion controller axis module.

The big book of Kinetix series cables is GMC-TD004.
