controllogix channel fault's meaning

That depends on the actual module, and how it's configured.
Some input modules can be configured to monitor wire-break (both analog and digital), and can have out-of-range alarms.
Output modules can monitor the on-board fuse or electronic trip circuitry.

The manuals are the best resource for interpreting the module status information.
