Honeywell DCS PMIO Analog Output Fault

We are facing problem related to honeywell DCS, Process Manager IO Analog Output Module (MC-PHAO01). While running it goes to NO RESPONSE state resulting plant to trip. After reinserting the module, it works fine. We also checked by replacing the module but even on new module this problem occur. the frequency of this erros is 3 times per year.

There is no cause or corrective action written in knowledge builder. Need to pin point the problem.
