Hi,
Just wanting to know if anybody out there uses some form of "change request" document that needs to be filled out by whoever is requesting the change, reasons behind the change, approval signatures etc.
I am carrying out a lot of minor changes, but feel that some documentation should accompany this rather than an email or verbal conversation.
