S7 Safety - QBAD_I_xx Not Turning Off

I have a strange problem.

We have 6 cart, all setup, wired exactly the same.
On two of those the QBAD_I_xx does not turn off.

The safety card is a 4/8 F-DI DC24V and it is connected to a HMI Mobile Panel --> 6AV2125-2GB23-0AX0, the enable and estop button.

We swopped out HMI's, cables and cards with no luck.
The device works as it should, estop and enable button working as it suppose to.

Any advice would help a lot
