Voltage and Current sensors

I am looking for a Voltage sensor that could give me feedback for voltage between 0 and 250 VDC with an analog output, either 0-10V or 4-20mA. And a Current Sensor for 0-12 Amps of DC current, also either 0-10V or 4-20mA.

Can anybody make any recommendations?
