SLC 5/04 Fault

The processor has an immediate fault light when power is applied. The processor is good and the power supply is good. Does anyone have or know how to get a pin out for the 1746-A13 chassis or the SLC 5/04?

A "pin out for the 1746-A13 chassis" ?
Are you wanting to troubleshoot the PLC by hooking a scope on the backplane bus ?

I am guessing you are asking how to connect to the serial port on the CPU.
For that you need a null-modem cable. Or make you own cable with the pins connected 2-3, 3-2, 5-5.
When you go online with RSLogix, you should configure a serial "RS232 DF1 device" driver in RSLinx. Use the "autoconfigure" button in the configuration dialog to automatically sense the baudrate etc.
Then when online in RSLogix go to the "Processor Status" and investigate the Major Error status word S:6.

What is the exact LED sequence?

You realize it's normal for a SLC to fault when it's (missing or different?) program does not match the I/O config.

Follow Jesper's wise advice and get online to check the fault code and the status of your SLC program (make sure it's really what you expect to have loaded).
